Retinal Vein Occlusion

Overview
Retinal vein occlusion (RVO) is one of the most common retinal vascular disorders, caused by thrombotic obstruction of a retinal vein that leads to hemorrhage, macular edema, and, in ischemic cases, neovascularization. It is classified as branch or central depending on the site of occlusion. Vision loss most often results from macular edema, which is treated effectively with anti-VEGF therapy.

Classification and risk factors
RVO is divided into branch occlusion, affecting a tributary vein at an arteriovenous crossing, and central occlusion at the level of the optic nerve. Each can be non-ischemic or ischemic; ischemia raises the risk of neovascularization. Associated systemic factors include hypertension, hyperlipidemia, diabetes, and, in younger patients, hypercoagulable states.
Diagnosis and workup
Fundus examination shows retinal hemorrhages in the distribution of the occluded vein, dilated tortuous vessels, and often macular edema. OCT quantifies edema, and fluorescein angiography characterizes perfusion and ischemia. A targeted systemic evaluation addresses modifiable cardiovascular risk factors, coordinated with the patient’s primary care team.
Treatment
Intravitreal anti-VEGF therapy is first-line for macular edema from both branch and central RVO and can improve vision substantially. Corticosteroid implants are useful alternatives, particularly in selected patients. Panretinal photocoagulation is applied when neovascularization develops, and ischemic central RVO warrants vigilant monitoring for anterior segment neovascularization and neovascular glaucoma.
