A stable tear film matters long before a patient reaches the operating room. In this clip from Episode 2 of Coffee with Nate, Dr. Nathan Radcliffe and Dr. Sumitra Khandelwal discuss how meibomian gland dysfunction and rapid tear break-up can affect refractions, contact lens fittings, glasses prescriptions, surgical measurements, and ultimately, patient satisfaction. When the ocular surface is unstable, even a routine refraction can lead to inaccurate results, costly remakes, and another visit for the patient. “You can’t really get the best refraction, the best contact lens, the best glasses, or the best surgery if you’re not treating that tear film stability.” The challenge becomes even more important when a patient is ready for cataract surgery but reports that their vision comes and goes. The cataract may still need to be addressed, but optimizing the ocular surface first can help produce more reliable measurements, better outcomes, and a more comfortable patient.
